The four routes
I have addressed it, verify: re-run the checks against the same document. It only closes when the gap is actually gone, so a fix has to be real to clear.
Assign to a person: hand the item to the counselor or clinician responsible for the document, so it lands with whoever can actually fix it.
Merge into another problem: attach this finding to a related one when they are really the same underlying issue, so you are not working the same thing twice.
Dismiss as minor: stop flagging one specific check for this patient and pattern. Even this is logged for audit.
There is deliberately no way to make a finding quietly disappear. Verify re-checks the evidence rather than taking your word for it, and even a dismissal leaves a record. That is what keeps your audit trail complete and your team honest, without slowing anyone down.
