What does breached mean?
A document has one or more checks that are still not met after their deadline has passed. Breached items are the ones that need you to resolve or route them.
What is the difference between not met and breached?
Not met means a specific check did not find what it required. Breached means a not-met check has also passed its deadline. Not met is a gap; breached is a gap that is now overdue.
Why is status descriptive instead of a pass or fail?
Adentris reports what it read and cites the evidence, but it does not declare a chart compliant, because that judgment belongs to a reviewer. Keeping status descriptive is what makes findings honest and defensible.
What is a control, a rule pack, and a zone check?
A control is a single check. A rule pack is a bundle of controls for a standard, level of care, or payer. A zone check is a control that reads across the whole chart rather than a single document.
Does a finding mean my chart is wrong?
Not necessarily. It means a required element was not found, or a code may not match the documentation. You review the evidence and decide what it means for your situation.
How often does Adentris review?
